Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our responsibilities

Newman Family Therapy Services is required by law to maintain the privacy and security of your protected health information, provide you with this notice of our legal duties and privacy practices, and notify you if a breach occurs that may have compromised the privacy or security of your information.

We will follow the duties and privacy practices described in this notice and give you a copy upon request. We may change the terms of this notice, and the new notice will be available upon request and on this website.

How we may use and share your information

We may use or share your protected health information for treatment, payment, and health care operations. For example, we may share information with other professionals involved in your care, use information to bill or receive payment from a health plan, and use information to manage and improve our practice.

We may also use or share information when required or permitted by law, including for public health and safety matters, health oversight activities, legal proceedings, law enforcement, workers’ compensation, and government functions. We will obtain your written authorization for uses or disclosures not otherwise permitted by law, and you may revoke an authorization in writing as allowed by law.

Your rights

  • Get a copy of your paper or electronic medical record.
  • Ask us to correct your medical record.
  • Request confidential communications, such as contact at a different address or phone number.
  • Ask us to limit certain uses or disclosures.
  • Get a list of certain disclosures of your information.
  • Get a copy of this notice and choose someone to act for you when legally authorized.
  • File a complaint if you believe your privacy rights have been violated.

Acknowledgment

You will receive this Notice of Privacy Practices no later than your first therapy session. We will make a good-faith effort to obtain your written acknowledgment of receipt.
